Participation is simple and you could potentially help create better future practices for all PMD children. If you would like your child/children to be included all you have to do is submit their IEP(s) and psychoeducational evaluation(s) along with the signed Informed Consent and HIPPA release forms. Those forms can be signed electronically, if you would prefer to do that please fill out the form below and we will email you the necessary information. For those that would prefer to print and/or save a copy they can be downloaded HERE.

Things the team has learned that can help:

• Many times IEPs and psychoeducational evaluations are double sided, please make sure to include both sides.
• IEPs and psychoeducational evaluations are two separate documents, both are needed to be included in the study. (Many states might summarize the evaluation information in the IEP, but it is incomplete, others do not include this at all.)
• Make sure that you read and complete all of the information on both the Informed Consent and the HIPPA Release form. (If sending  IEPs and psychoeducational evaluations for multiple children please keep in mind that you will need to send an Informed Consent and the HIPPA Release form for each child.)​
