Please make sure you download both the Informed Consent form and the HIPPA Release form. Make sure that you read and complete all of the information on both forms. (If sending IEPs and psychoeducational evaluations for multiple children please keep in mind that you will need to send an Informed Consent and the HIPPA Release form for each child.
